Request Your Medical Records Online

  • Patients: Click on the “Begin” button below and fill out the form to request your medical records. You will be asked to provide a copy of your ID as part of the process.
  • Third Party Request: Click on the “Begin” button below, fill out the requested details, and upload your request.
  • Note: You may be contacted to provide additional documentation, such as a birth certificate, power of attorney, etc. You will receive an update or response from Sharecare within 3 business days.
